Immunization Program
We provide immunizations for West Haven (residents only) children from newborn to high school age.
Registration:
Parents should call the health department and make an appointment with a nurse to come in and complete the registration forms. Parent must know which immunization child needs. If they have a letter from the school nurse or physician they must bring it with them so the nurse will know what shots the child needs. The shots will be given at our medical advisor's office. Our medical advisor is Dr. Umapathy and is located down the street from the West Haven Health Department on Main Street. The parent will received documentation of immunizations for their records as well as for the school nurse/physician. If follow up visits for further immunizations are required, the nurse will advise the parent when he/she should call the health department for an appointment.
